Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a recognized electrical shock or burns:
(a)M/V Polar Queen:  On the main deck, conductors were exposed on the fish
pump
switch.  This condition exposed employees to possible electrical shock or
burns
in the event of accidental contact.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard would be to replace the pump switch.
(b)M/V Polar Queen:  In the engine room, two fluorescent lights fixtures
less than
7 feet were not guarded to protect the tubes from accidental contact.
One economical and feasible method of abatement would be to install
approved
tube guards.
(c)M/V Polar Queen:  In the freezer hold, an electrical junction box was
missing a
cover.  This condition exposed employees to electrical shock or burns.
Among other methods, one feasible and economical abatement to correct this
hazard would be to replace a junction box cover.
(d)M/V Polar Queen:  In the freezer hold on the ceiling, a rectangle
electrical wire
box was missing the cover.  This condition exposed employees to possible
electrical shock or burns.
Among other methods, one feasible and acceptable abatement methods to
correct
this hazard is to replace the cover.
(e)M/V Polar Queen:  In the forward pump room, the main panel breaker had
an
unused opening.  This condition exposed employees to possible electrical
shock
in the event of accidental contact while flipping switches at the panel
box.
Among other methods, one feasible and economical method to correct this
hazard
would be replace the missing circuit breaker.
(f)M/V Polar Queen:  In the machine shop, three 4 foot fluorescent light
fixtures
were missing bulbs and covers.  This condition exposed employees to
possible
electrical burn or shock in the event of accidental contact.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard would be to replace the tubes and install approved light
guards.
(g)M/V Polar Queen:  In the forward pump room, one 4-inch round electrical
junction box was missing a cover.  This condition exposed employees to
electrical
shock or burn in the event of accidental contact with the conductors.
Among other methods, one feasible and economical abatement to correct this
hazard would be to replace the cover.
