Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the recognized hazard of exposed live electrical parts:
a) M/V Palisades:  The breaker box located in the engine room used to
supply power to the services in the engineering spaces was not
equipped with a dead front to protect employee(s) from contacting
the live 220 volt bus bar.  This condition exposed employee(s) to
electrical shock and/or electroction.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is close opening with a dead front to prevent
accidental contact with energized parts.
b) M/V Palisades:  The breaker box located in the engine room supplying
power to the aft end of the vessel for the clothes dryers, bilge
alarm, and heaters, was not equipped with filler blocks, exposing
employee(s) to contacting the live electrical bus bar. This condition
exposed employee(s) to electrical shock and/or electrocution.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is, close openings with filler blocks to prevent
accidental contact with energized parts.
