Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) 128 N. McKean Street, Butler, PA 16001; Employees were exposed to fall hazards up to approximately 55 feet when entering/leaving the platform above ground in that the platform of the JLG Lift Model 60H was not within one foot of the adjacent roof jack platform. Among other methods, one feasible and acceptable abatement methods to correct this hazard is to position the platform of the JLG Lift Model 60H within one foot of the adjacent roof jack platform, allowing for platform vertical movement when entering or leaving the platform.

McKean Street, Butler, PA, 16001; Employees were working on the roof without the use of fall protection thereby exposing employees to a fall hazard up to approximately 35 feet. Among other methods, one feasible and acceptable method to correct this hazard is the use and enforcement of safety belts/lanyards/lifelines.
