Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The

employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that Boiler House employees were
not adequately protected against hazardous concentrations of carbon
monoxide from leaking coke oven gas while working throughout the boiler
house building:
a)  Boiler House:  The #10 water tender and #10 water tender helper
were potentially exposed to high levels of carbon monoxide and
were not provided with an adequate alarm and warning system.
Employees are potentially exposed to carbon monoxide while
working throughout the boiler house building (approximately 220
ft. x 50 ft.).  The fuel used in the boiler house is coke oven
gas which contains 4% carbon monoxide.
Feasible and useful abatement methods for reducing this hazard, among
others, are:
1.  The installation of permanently installed carbon monoxide
alarms at various locations on both floors of the boiler
house building.
2.  Personal carbon monoxide meters with audible alarms to be
carried by workers when responding to and repairing coke
oven gas leaks.
