elevator was not installed in accordance with American National Standards
Institute Safety Code for Elevators (ANSI A17.1-1955).  See appropriate

below:
1.The hoistway was not fully enclosed.  See rule 100.1c.
2.The elevator car was not fully enclosed.  See rule 204.3.
3.The elevator was not equipped with a car door.  See rule 204.4.
4.The elevator was not equipped with an emergency stop switch.  See rule
210.2e.
Note:  A operating switch will be required inside the car.
(b)Feed Mill:  On or about 2/8/96 and at times prior thereto, the Otis
elevator,
serial number 229735, was not installed in accordance with American
National
Standards Institute Safety Code for Elevators (ANSI A17.1-1937).  See
appropriate rule below:
1.The hoistway was not fully enclosed at the basement level.  See rule 101.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is to:
1.  Repair elevator in accordance with current ANSI A17.1 series.
