The alleged violations below have been grouped because they involve similar
or related hazards that may increase the potential for injury resulting
from
an accident.
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death or
serious physical harm to employees in that employees were exposed to a
recognized hazard of electrical shock or burns:
(a)   M/V Yardarm Knott:  Employees working and walking in the
chiller
flat room had occasion to be exposed to an electrical switch
that the ground bypassed by an extension cord.  This condition
exposed employees to possible shock and/or burns should they
become a source to ground.
Among other methods, one feasible and acceptable method of
abatement would be to install an electrical outlet within the
reach of the switch.

recognized hazard of electrical shock:
(a)   M/V Yardarm Knott:  Employees in the chiller flat room had
occasion to walk and work near an exposed, unguarded electric
incandescent light bulb that was located at normal head
height.
This condition exposed employees to a possible broken bulb and
contact with live electrical conducts.
Among other methods, one feasible and acceptable method of
abatement would be to install/replace the guard.

had occasion to work with a hydraulic hose crimper that had
the
protective wafer missing on the plug-in.  This condition
exposed
employees to a shock and burn hazard.
Among other methods, one feasible and acceptable abatement
method is to install a dead fronted plug-in.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death or
serious physical harm to employees in that employees were exposed to a
recognized hazard of personal injury of broken bones, lacerations, and
death.
(a)   M/V Yardarm Knot:  Employees were observed in the freezer
storage areas working around and near an elevator/hoist that
did
not have the bottom landing for the elevator hoist protected
or
guarded to prevent employees from placing themselves in a
position of danger under the elevator/hoist during the
descending operation of the elevator/hoist to the freezer
storage deck.  This condition exposed employees to personal
injuries of broken bones, lacerations and death should they
become trapped between the elevator/hoist and the deck
landing.
One feasible and acceptable method of abatement would be to
install appropriate guarding around the front and sides of the
landing and insure employee compliance of keeping guards in
place during operation of the elevator/hoist.
Yard Arm Knott, Inc. was previously cited for a REPEAT
violation
of this Occupational Safety and Health standard or its
equivalent standard which was contained in OSHA Inspection
Number 107107476, Citation Number 2, Item Number 5, issued on
September 27, 1990 and OSHA Inspection Number 107294340,
Citation Number 1, Item Number 4, issued on September 7, 1988.

engine room had occasion to be exposed to
uncovered/unprotected
electric panel bus bars in a panel box near a normal walkway.
This condition exposed employees to possible electrocution,
electric shock and/or electrical burns.
Among other methods, one feasible and acceptable method of
abatement would be to replace the panel covers and deadfront
the openings to the bus bar.
Yard Arm Knott, Inc. was previously cited for a violation of
this Occupational Safety and Health standard or its equivalent
standard which was contained in OSHA Inspection Number
107107476, Citation Number 1, Item Number 1, issued on
September
27, 1990.
