employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
falls from an elevation where the load imposed on the manila rope being
used to suspend the scaffold exceeded the safe working load of the rope.
a)  N. J. side of the Walt Whitman Bridge, the 1st of nine
two point suspended scaffolds on the West side of the Jersey
Tower and North side of the bridge roadway.  The scaffold
was occupied by two employees engaged in blasting/painting
operations on the bridge's main suspension cable.  The
scaffold was approximately 365 feet above the adjacent ground/
water level.
Among other methods one feasible and acceptable method to correct
the hazard is to place the scaffold platform on a safe surface.

did not employ a thimble nor was the eye of the wire rope secured by "J"
clamps properly torqued.
a)  Walt Whitman Bridge, New Jersey side of the bridge West
side of the New Jersey Tower, north side of bridge.
Scaffoldings
occupied by employees that were supported by wire ropes
reeved through Sky Climber hoists and then looped around the
bridges main support cable and fashioned into an eye and
secured to itself.
Among other methods, one feasible and acceptable method to
correct the hazard would be to secure the wire rope to a
choker that would be looped around the main suspension cable.
The eye formed in the wire rope would then be fitted with a
thimble and the eye on the wire rope would be secured by a
"J" clamp properly installed and torqued as per manufacturer's
recommendations.

Among other methods, one feasible and acceptable method to correct
the hazard is to install Sky Lock Overspeed Brake Safety Devices
on each of the eighteen (18) Sky Climber Hoisting Machines being
used at the site.
