Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees,in that,employees were exposed
to serious fall hazards from the second floor level and serious crushing
injuries on the first floor level.  A feasible and useful method to correct
the hazards among other methods, is to provide electrically and
mechanically interlocked gates to prevent them from being opened until
the platform has reached the level:
(a)  Warehouse:  The giant lift inclined reciprocating conveyor was
not equipped with mechanically interlocked gates exposing employees
to serious fall hazards and crushing injuries.  The gates were not
of sufficient height to prevent employees from inadvertantly falling
over the gates on the second floor level.
