Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees, in that, employees were exposed
to serious fall and crushing hazards:
(a)  Beater Room Area:  Employees were exposed to serious fall hazards
from the second floor approximately a 5 feet 4 inch drop and serious
crushing injuries on the first floor level from the vertical
reciprocating conveyor transmitting materials from the first floor
level to the second floor level.
A feasible and useful method to correct the hazards, among other
methods, is to provide electrically and mechanically interlocked gates
to prevent them from being opened until the platform has reached the
level.  In addition to installing a gate around the conveyor when
located at the second floor level so to prevent employees falling
under the platform.
