An emergency lighting system was not provided in elevators #1, 2, 3 and 4 that would be automatically turned on in all elevators in service immediately after normal car lighting power failed. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: 1) Provide an emergency lighting system for all elevators in service. Emergency lights shall be automatically turned on immediately after normal car lighting power fails. Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnsih employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Employee(s) were exposed to the hazard of being trapped in elevators #1, 2, 3 and 4 without a means of an emergency alarm system during an emergency such as fire, injury, mechanical failure, etc. An emergency signal to alert personnel of an emergency and a means for a two-way conversation between each elevator at a readily accessible point outside the hoistway was not provided. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: 1) Install an audible signaling device, operable from the emergency stop switch and from a switch marked "ALARM" which are located in or adjacent to each car operating panel. The signaling device shall be located inside the building and audible inside the car and outside the hoistway. One signaling device may be used for a group of elevators. 2) Install a means of two-way conversation between each elevator and a readily accessible point outside the hoistway (telephone, intercom, etc.) when employees are left unattended. Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: On or before June 1, 1990 to on or about July 16, 1990, employees operating a freight elevator, and/or riding a freight elevator, and/or working in the vicinity of a freight elevator were exposed to the hazard of falling into the elevator shaft and/or being crushed by the elevator cab. a) Location: Elevator #4, Floor 6, Door - B Among other methods, one feasible and acceptable abatement method to correct this hazard is to: 1) Equip each hoistway door with a hoistway door interlock device. 2) The interlock device shall hold the door in the locked position by means of gravity or by a restrained compression spring, or by both, or by means of a positive linkage. 3) The interlock shall lock the door in the closed position before the elevator can be operated.
