Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to burning:
(a)  Plant 3:  The hazard of severe burns resulting from possible
rupture of a pressurized vessel used to contain tantulum in an
experimental process on October 9, 1992.
Among other methods to correct this hazard, is development and
implementation of a procedure whereby experimental processes
will only be attempted after all such procedures have been
documented, reviewed and approved by someone qualified in process
design.
