Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
a.  On May 31, 1991, at approximately 11:15 p.m., the driver
(employee) rotated the discharge auger(boom) and lowered it into the
electrical distribution (transmission) lines which was rated at 7200
volts.  The driver (employee) after attaching a booth, a flexible
chute, to the end of the discharge auger (boom) raised the discharge
auger(boom) up to the top of the feed bin and unloaded the feed from
the bulk feed trailer.  The driver (employee) then rotated the
discharge auger (boom) to the left and to the rear of the bulk feed
trailer after unloading the feed at the M&M Poultry Farm on Township
Road 167, Sugarcreek, Ohio resulting in the truck driver (employee)
being electrocuted.
Among other methods, one feasible and acceptable abatement
method to correct this hazard is to :
a.  Identify all delivery areas where there are electrical
power lines in close proximity  to where the feed is to be unloaded;
b.  After identifying where the electrical power
lines are located, provide each driver (employee) with insulated
blankets, and instruct and train in the use of the insulated blankets;
c.  Avoid making deliveries to those areas that have been
identified as to having overhead electrical power lines at night.
