injury from an unintended actuation of a foot pedal, not protected by a
guard:
(a)Press Department - An employee operated a Wysong Press Brake, model
90-10, serial
number PB 56 - 178, to punch and notch pieces. No means was provided to
protect
the foot pedal from an unintended actuation by a falling or moving objects
or by
someone accidentally stepping on it.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD
TO CORRECT THIS HAZARD IS TO GUARD THE FOOT PEDAL.
