General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical bodily harm to employees in that employees were exposed to the potential hazards of unexpected energization or start up of machines or equipment or release of stored energy. Employees performing maintenance, set-up, repairs, or calibration of machine tools were not provided with task specific lock-out/tag-out procedure. One method to correct this apparent hazard would be to develop task specific lock- out/tag-out procedures to ensure the control of hazardous energy.
