Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of electrocution or electrical shock or burns:
(a)M/V Pacific Producer, aft end of the freezer tunnel:  On the freezer
deck, a
duplex receptacle box was missing its cover.
(b)M/V Pacific Producer, aft end of the combination level:  The cord
powering the
lights was run through a hole in the lip of the metal deck above.
Among other methods, one feasible and acceptable abatement method to
correct
these hazards would be to replace the electrical outlet cover plate and
protect the
electrical cords from abrasion.
(c)M/V Pacific Producer, refer flat:  A 480-volt electrical panel did not
have a
minimum of three feet front working space and access to the panel was
blocked.
(d)M/V Pacific Producer, forward bow:  A 220-volt electrical panel did not
have
a
minimum of three feet front working space and access to the panel was
blocked.
Among other methods, one feasible and acceptable abatement method to
correct
these hazards would be to clear access to the electrical panels and
maintain a
three feet working space around them.
(e)M/V Pacific Producer:  The employer's lockout/tagout program did not
specify
that lockout/tagout devices were to be used only for controlling energy.
In
addition, there were not lockout/tagout procedures for each individual
piece of
machinery or equipment which had more than one source of energy or the
potential of stored energy.  This condition exposed employees to possible
electrical shock or burns in the event all energy sources were not
isolated during
machinery and equipment repair.
Among other methods, one feasible and acceptable abatement method to
correct
his hazard should be to acquire appropriate lockout/tagout devices and
develop,
document, and utilize energy control procedures for each individual
machine or
piece of equipment.
