Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of electrocution: (a)M/V Northland: The reflectors of two clamp on lights in the egg room were not grounded. Among other methods, one feasible and acceptable method to correct this hazard is to ground the metal reflectors. (b)M/V Northland: The extension cord from the LPR room had been run through a door and showed signs of being pinched. When the cord was tested, it showed no ground. Among other methods, one feasible and acceptable method to correct this hazard is to replace the cord and relocate it so that it is not subject to pinching. (c)M/V Northland: Procedures were not developed for an individual machine or piece of equipment which had more than one energy source or the potential of stored energy. Among other methods, one feasible and acceptable method to correct this hazard is to provide procedures for multiple energy source equipment. (d)M/V Northland: No locks were provided for use in isolating equipment and all the tags provided had been used before replacements were purchased. Among other methods, one feasible and acceptable method to correct this hazard is provide tags and locks. (e)M/V Northland: In the engine room, a 110-volt electrical panel had two uncovered breaker (overcurrent protection device) openings that exposed employees to possible contact with live electrical conductors. (f)M/V Northland: In the LPR room, the cover was missing from a 110-volt electrical outlet, exposing employees to the hazard of electrical shock. Among other methods, one feasible and acceptable method to correct these hazards is to repair the missing breakers and outlet cover.
