recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to hazards due to the absence of a hoistway-door locking
device:
a)The hoist in the Production Department, between the Cutting Department
(second floor)
and the Sewing Department (first floor), does not have a hoistway-door
locking device
to prevent operation of the hoist if the door is open, and to prevent
opening of the door
unless the hoist is at the landing.  The absence of a hoistway-door
locking device places
employees at risk of becoming crushed between the hoist and shaft and/or
falling into the
shaft, resulting in broken bones and/or amputations.  One feasible method
of correcting
this hazard is to install a hoistway-door locking device as referenced in
ASME A17.1B,
1995, Section 700, Hoistways, Hoistway Enclosures, and Related
Construction, Rule
700.12 Hoistway-Door Locking Devices, Access Switches, and Unlocking
Devices.
