Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazard of being struck by the load line wire rope and snatch block
(a whip lash action) where a base mounted hoist is used for horizontal
pulling of the slasher deck scraper car.  One feasible and acceptable
method to correct this hazard is to comply with proper rigging where
employees are required to work within the bight of the wire rope and
snatch block:
(a)  The wire rope on the hoist is crushed and kinked on the
base mounted hoist drum.  This line must be replaced
(b)  The eye at the end of the wire rope which is attached to
the scraper car is formed with knots, u-bolt and clips.  Eyes
shall be formed by approved wire rope eye forming devices and
knots in wire rope cannot be used.
(c)  The wench operator shall be shielded behind a barrier to
prevent exposure from whip lash if the wire rope breaks.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazard of smoke inhalation and thermal burns due to backdraft of fire
and smoke from the bark boilers at the #1 bark boiler,ground floor at
the chutes to the bark boiler (confined space) to use an air lance with
compressed air to force bark into the boiler which has jammed or plugged
the feed chutes.  They are required to open the inspection ports
or doors of these bark feed chutes and blow the plugged bark into the
boiler furnace.  This action causes burning bark and flames to blow
back onto them with heavy smoke.
Among others, one feasible and acceptable method to correct this hazard
is to install the inspection ports or doors on the front side of the
bark feed chutes which would be at a downward angle, so that burning
bark, fire and smoke would not fall out of the bark feed chutes onto
employees.  Also, employees will have an adequate escape route from
this side.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazard of thermal burns from hot smelt spitting from the boiler
smelt spouts.  This condition occurs when the boiler is sucking too much
air causing the smelt to cool, lump up and plug the smelt spouts.  When
this condition occurs,  employees were allowed to go to the boiler at
the smelt spouts while it is spitting hot smelt to shut off the fuel
supply (black liquor) by a manually-operated valve, and to remove the
black liquor fueling nozzle/gun which exposes employees to the hazard
of being burned by the hot smelt.
Among others, one feasible and acceptable method to correct this hazard
is to relocate the manual shut-off valve away from the boiler smelt
spouts and to leave the fuel nozzle/gun in place.
Additionally, written operating procedures should be developed and
distributed to each affected employee outlining the procedures to take
during a boiler blow-back.

#1 recovery boiler, 8th floor at southside man-door.  The man-door latch
is broken and the door is jammed with a steel rod.  The steel rod does
not keep the man-door completely closed which allows steam and ash to
escape from the door openings.
Among others, one feasible and acceptable method to correct this hazard
is to repair the latch on the man-door to insure that the door
effectively closers as intended by engineering and design.

mately five feet in diameter and weighing 800-1000 pounds each.  The saw
blades are stored in a "saw rack", and when the rack is open employees
are exposed to being struck by saw blades should the blades roll out of
the rack.
Among others, one feasible and acceptable method to correct this hazard
is to re-construct a saw-rack" that will contain the circular saw
blades when the saw rack gate is open, thus preventing the circular
saw blades from rolling out of the rack onto employees.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazard of being struck by falling logs.  Employees are required
to climb into the debarker frum to unchoke the debarker where logs are
sometimes cut too long and the log will try to go through the debarkers
circular opening in a vertical position causing a choke condition.
Other logs will pile up behind the choke log.  When this condition
occurs, employees must go into the debarker to wrap a choker sling
around the choke log to feed it.  The debarker is then energized,
and as it makes a revolution, the choke log is snatched free.
Among others, one feasible and acceptable method to correct this
hazard is to install an air tugger or winch somewhere above the
debarker opening.  An eyelet or other type pick-up device can be
installed at the end of the winch load line and a pickroom or
other long pole device can be used to pull the wire rope around
the choke log back up to the operator.  This method would not
require an employee to enter the debarking drum to unchoke it.
A special written operating procedure should be done to initiate
employees with this procedure.

the hazard of falls into chipper knives and logs 22 feet into the
infeed chute of the chipper.  Employees use a fiber rope to lower
themselves down the chipper chute into the base of the chipper to
unclog or unjam it.
One feasible and acceptable method to correct this hazard is to
construct a swing and removable beam over the chipper chute where
a control descent device can be hung or supported.  The employee
shall use a body harness hooked to the control descent device and
he can be lowered into the infeed chute of the chipper.  The
chipper shall be locked out in accordance with lockout/tagout
procedures.

with concensus standards, American National Standard Institute, Mobile
Hydraulic Cranes, ANSI B30.15-1973, Section 15-1.9.3(a):  Means were
not provided when blocked for hoisting.
