Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to the hazard of a collapse of a twopoint
suspended scaffold:
a.ON THE JOBSITE, SYRACUSE UNIVERSITY, SOUTH SIDE OF GGERS
HALL.  On or about September 15, 1993.  Two (2) employees
were exposed to a fall hazard while using a HitLo
Climbers, model D800 Series twopoint suspended
scaffolding while installing caulking material along the
outside masonry wall seams.  The following rigging
procedures were not followed as required by the
manufacturer of HiLo Climber; and the American National
Standard Institute (ANSI A1081988) on Scaffolding
Safety Requirements.
1.A Torque Wrench reading FTLBS t determine that the
5/16 inch first grip wire rope clips were tightened
to 30 FTLBS onto the wire rope suspending the two
point scaffold.
2.Applying a proof load to the wire rope to check
first grip clips and then retorquing the clips to
30 FTLBS with the proof load applied.
One feasible and acceptable abatement method, among others, to
correct these hazards are:
1.Using a torque wrench which reads in FTLBS to
adequately torque the fist grip wire rope clips to
the wire rope according to the manufacturers
specifications of 30 FTLBS.
2.Applying a proof load of 1,000 LBS to the wire rope
to check the fist grip clips and then retorquing
the fist grips again to 30 FTLBS with the proof
load applied.
