Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
Hazards of being crushed, struck or pinned.
Frame Shop:
In that employees were exposed to the hazard of being struck, crushed
or pinned from the two semi hydraulic automotive lifts while working
under vehicles on the lift, which was being operated with a defective
automatic locking device.
"AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE
ABATEMENT METHOD TO CORRECT THIS HAZARD IS...."
a) Obtain and conduct the recommended inspections of the lift by the
manufacturer to ensure that the automatic mechanical locking device
and all other operating components are in a safe operating condition.
Remove the defective lift from service until rendered safe for use.
b) Follow the requirements of ANSI B 153-1 1990, Automatic Lift
Institute
Publication ALISM 89-1 and NOISH Publication No. 77-22.
Automatic Mechanical locking device shall hold the lift in the
extended position.
c) Obtain and conduct required maintenance procedure described by
the manufacturer.  Inspect lift daily, never operate if it
malfunctions
or if it has broken or damaged parts.
d) Instruct and train employees in the proper use of the manufacturers
procedures for safe operation of the lift including the automatic
mechanical locking device.
