Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
(a)  6/14/93 at the jobsite, a freight elevator was in service
which had no rear car enclosure gate, no capacity plates,
no minimum load permitted, no top enclosure, no internal
lighting, and no maintenance inspection certificate or
service test certificate as required by ANSI/ASME
A17.31986
and BOCA 1984.  Employees were exposed to serious
fall injuries should the elevator fall down the shaft or
employee falls out the back side.
Feasible and acceptable methods among others to correct the hazard is to:
1) Comply with ANSI/ASME A17.3-1986 and BOCA 1984.
2) Conduct periodic inspections of elevator as recommended by the
ANSI/ASME
& BOCA Standards to insure integrity of interlocks,ropes & switches
for gates and equipment, & document above.
3) Post minimum & maximum load rating on capacity plates as recommended
in standards.
4) Fully enclose the freight elevator car as recommended by standards.
5) provide internal illumination as recommended.
A search of the OSHA Standards has been conducted and there are no
applicable standards.
