Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
(a)  Foundry - the mold cool down house had not been identified as a
confined space by the employer.  Maintenance employees enter the sold
cool down house on an as needed basis to keep the conveyor system on
track.  Mold gases and fumes are potentially excessive during this
time.  Safe entry procedures for a confined space were not followed.
Among other methods, feasible and acceptable means to correct this
hazard include:
1.  Improve existing machanical ventilation system by redesign of
makeup
air and exhaust requirements.
2.  Develop an entry "permit" system.
3.  Designate a qualified person to perform atmosphere testing prior to
entry and during entry.
4.  Establish a "buddy" system so that an attendant is available to
monitor the entry.
5.  Provide rescue equipment (i.e., a self contained breathing
apparatus).
6.  Provide employee training covering all aspects of the program.
