Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burns, falls, and explosion hazards due to the escape system not being in place: a) On or about April 20, 2007 and at times prior, Eastern Colorado Well Service LLC did not ensure that employees were protected from fall, fire, and explosion hazards due to the geronimo line not being installed at the monkeyboard. One feasible and acceptable method of abatement would be to train the crews to inspect and properly install the excape device before begining well servicing activities. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").-Certification

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushing hazards from falling derricks or rotating tongs which were improperly connected to their wire rope anchor/guy lines: a) On or about April 20, 2007 and at times prior, Eastern Colorado Well Service LLC did not ensure that employees were protected from struck-by and crushing hazards. 1. Crosby clips were not used correctly in that the "U" section was placed in contact with the live end of the wire rope, thus crushing and stressing the wire strands. 2. Crosby clips were not used correctly in that the correct number and spacing for the size of wire rope according to ASME B30.9 was not followed. 3. Thimbles were not used at points of connection leaving wire rope to spreading apart and abuse to individual wire strands. 4. Job built anchor clamping devices were not tested to ensure they conform to manufacturers specifications on rig anchorage. The slots for the chain to connect to is not uniform in size and placement. The "U" section of the crosby clamps used is stressing the wire rope. 5. Load boomers were used to tighten the wire rope cables for the rig and then were left with the tension of the line intact. Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow wire rope manufacturers recommendations Ref: ASME B30.9. Job built clamping devices and unrated devices should not be allowed to be used as part of the anchoring process unless they are tested and rated to accepted engineering standards.
