a)Meitetsu Mart: On 04/09/02 an employee standing on an unsecured wood
pallet without
standard guardrails was raised twelve (12) feet above the concrete ground
by a Toyota
forklift vehicle(CNMI vehicle license HE 1422) in order to change light
bulbs in the
Meitetsu Mart sign. The employee was not provided with a safety cage or
other safe
means of being lifted or elevated.
Note: Among other feasible and acceptable abatement methods would be to:
1.Provide a "safety platform" used in accordance with ANSI B 56.1 1975,
Part III-Section
513, or equivalent protection.
2.Provided a scaffold or solid construction from which to change the sign
light bulbs.
3.Provide a ladder.
