Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees involved in steel making processes on the Operating Floor of the Rokop Caster were exposed to fire and explosion where the natural gas hoses were not approved where they were used to connect the pre-heaters to the tundishes. Among other methods, one feasible and acceptable abatement method to correct this hazard is to use only approved hoses, as recommended by the manufacturer.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to potential amputations, where the gates on the freight elevator were not a minimum of six feet in height, in the following locations: 1.At the Demag Elevator, southwest side, first level. 2.At the Demag Elevator, southwest side, second level. 3.At the Demag Elevator, southwest side, third level. Among other methods, one feasible and acceptable method to correct this hazard is to insure the gates are installed in accordance with ANSI-A17.1, 1965, Section 204 (6)(b).

Among other means, one feasible and acceptable abatement method to correct this hazard is to equip all elevator doors with interlocks, in accordance with ANSI A17.1 - 1993, Rule 204.4(b).e
