Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the unexpected release of the cement bucket and/or its contents due to wire rope failure: COMPANY: EMPLOYEES WERE EXPOSED TO THE HAZARD OF BEING STRUCK BY/ CRUSHED BY THE SKIP HOIST BUCKET AND/OR ITS CONTENTS AND/OR BEING STRUCK BY THE WIRE ROPE ITSELF WHILE OPERATING THE SKIP HOIST PROVIDED ON THE CEMENT MIXER DUE TO THE LACK OF INSPECTION AND MAINTENANCE OF THE WIRE ROPES THAT LIFT THE BUCKET. ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD, AMONG OTHERS, IS TO: DEVELOP AND IMPLEMENT A PROGRAM OF REGULAR, DOCUMENTED INSPECTIONS OF THE SKIP HOIST COMPONENTS AND PERFORM MAINTENANCE AS INDICATED DURING THE INSPECTIONS.
