Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards due to uncontrolled movement and/or falling of loads: COMPANY: EMPLOYEES WERE EXPOSED TO THE HAZARD OF BEING CRUSHED AND/OR BEING STRUCK BY LOADS WHILE LOADING AND UNLOADING CONCRETE PRODUCTS USING A TRUCK MOUNTED MONORAIL CRANE AT THE SHOP AND AT VARIOUS INSTALLATION SITES DUE TO THE LACK OF THE DEVELOPMENT AND IMPLEMENTATION OF PROGRAM OF REGULAR DOCUMENTED INSPECTIONS AND PREVENTATIVE MAINTENANCE ON THE EQUIPMENT AND NOT DETERMINING AND ADEQUATELY INFORMING EMPLOYEES OF THE LIMITATIONS OF THE EQUIPMENT. ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD, AMONG OTHERS, IS TO: DEVELOP AND IMPLEMENT A PROGRAM OF DESIGN, INSTALLATION, TESTING, INSPECTION, PREVENTATIVE MAINTENANCE AND TRAINING THAT INCORPORATES ANSI B30.11 (MONORAIL SYSTEMS AND UNDERHUNG CRANES) AND B30.16 (OVERHEAD HOISTS) AND THE COMPANY'S EXPERIENCE USING THE EQUIPMENT.
