Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a potential crushing injury from a falling ceiling or roof rafter assembly with an inadequately engineered lifting device: (a)Ceiling Building Area - Eight (8) employees using a large spreader bar (ceiling set beam) to set the constructed roofs in place were potentially exposed to a crushing hazard. The "homemade" lifting device has not been adequately designed, marked with load rating, inspected periodically, or proof tested and certified, on or about April 17, 2002. Among other methods, one feasible and acceptable abatement method to correct this hazard is to build the ceiling and roof rafter system directly on the housing unit. VERIFICATION REQUIRED

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a potential crushing injury from a falling floor area with an inadequately engineered second support system: (a)Floor Building Area - Nine (9) employees were potentially exposed to a crushing hazard while working under a suspended floor unit weighing approximately 3022 pounds. The load was suspended overhead by two (2) CM two (2) ton hoist with four (4) inadequately designed chain support hooks, on or about April 8, 2002. Among other methods, one feasible and acceptable abatement method to correct this hazard is to eliminate working under the suspended load. VERIFICATION REQUIRED
