employees were exposed to a crushing hazard due to being struck by a
moving car conveyor or caught between the moving car conveyor and
equipment structures when the conveyor is manually started:
a)   Roto-Pour Area - The roto-pour car conveyor was not
provided with an audible and/or visual alarm device to
alert employees that the conveyor is about to start,
when operated from the remote override control station,
during cleaning operations.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to:
1.     Install a audible and/or visual alarm device which will be
activated when the manual override control station is
operated, that can be clearly heard and/or seen at all
points along the car conveyor.  Reference ANSI B.20.1,
Section 5.112(b)(1) 1987.
2.     Permit only trained employees to operate car conveyor.

employees were exposed to a crushing hazard due to being struck by a
moving car conveyor or caught between the moving car conveyor and
equipment structures when the conveyor is manually started:
a)   Roto-Pour Area - The roto-pour cooling structure was
not provided with a barrier guard on the West side
approximately 63 inches high and 126 inches wide to
prevent employee entry while the car conveyor was in
motion.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to:
1.     Provide barrier guards to prevent entry into shear points
along the car conveyor.  Reference ANSI B.20.1, Section
5.9.3, 1987.
b)   Roto-Pour Area - The car conveyor was not provided with
a barrier guard to prevent employee contact with moving
cars while setting core on the East side of the mold
line.

a)   Aisco Unit - The spill sand belt conveyor was not
provided with an emergency pull cord to stop the belt
conveyor in the event an employee was caught in the nip
points along the conveyor during cleaning operations
when the belt is in operation.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to:
1.     Provide an emergency pull cord along the entire length of
the belt conveyor.  Reference ANSI B.20.1, Section
5.11.2(c), 1987.
2.     When proper cleaning requires the conveyor to be in motion
and a hazard exists, personnel should be made aware of the
associated hazards.  Reference ANSI B.20.1, Section 5.12(j),
1987.
3.     Personnel working on or near a conveyor shall be instructed
as to the location and operation of pertinent stopping
devices.  Reference ANSI B.20.1, Section 5.12(e), 1987.

employees were exposed to crushing injuries due to unexpected shifting
of loads or falling hoist parts when lifting and moving material with
the overhead hoists:
a)   Quality Control Department - A jib hoist was
extensively repaired, not load tested, and the welding
was done by in house personnel.
b)   Patter Shop - The two overhead hoists were not
certified as to the capacity of the hoist beam and
supporting structures.
c)   Rock Pile Department - The overhead hoist was operated
with the hoist beam damaged, and not taken out of
service.
Among other methods, one feasible and acceptable abatement method to
correct his hazard is to:
1.     All clamps, hanger rods, bolts or other suspension fittings
and supporting structure shall be designed to support the
rated loads in accordance with specifications in the AISC
Manual of Steel Construction.  Reference ANSI B.30.11-1973,
Section 11.1.3.2.
2.     All welding shall conform to the recommended practices of
the American Welding Society as outlined in AWS D14.1 for
Industrial and Mill Cranes. Reference ANSI B.30.11-1973,
Section 11.1.3.4.
3.     Prior to initial use, all new, extensively repaired, or
altered equipment shall be tested by or under the direction
of an appointed person and a written report furnished by
such person, confirming the load rating of the system.
Reference ANSI B.30.11-1973, Section 11-2.2.2.
4.     An unsafe condition disclosed by the inspection requirements
of Section 11.2.1 shall be corrected before operation of the
underhung crane or monorail system is resumed.  Reference
