(a) Location - Jobsite, exterior of 90 Pleasant Valley St., Methuen, MA:
On oar about 4/13/00, two employees were exposed to a serious fall hazard
of 29 feet
while utilizing a 4 foot step ladder within the platform of the
Shaughnessy Sizzor Lift to
reach an upper level.
Among other methods, feasible and acceptable to correct this hazard
include, but are not
limited to:
- Extend the Sizzor lift up to the roof's edge so as to utilize the access
gate from the
platform.
- Read and follow the operator's manual guidelines for safe use.
