Section 88.4 - Code of Iowa (2001) 88.4; Reference ANSI MH16.2-1984, Section 2.2: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of not knowing the manufacturer's intended procedures to safely erect and dismantle industrial steel storage racks: (a)Warehouse - Manufacturer's instructions were not available for employees to review prior to erecting and dismantling industrial steel storage racks. On 11/5/01, employees were dismantling an industrial steel storage rack without previously being able to review the manufacturers instructions causing an accident which resulted in an amputation injury. Among others methods, one feasible and acceptable abatement method to correct the hazard is to obtain manufacturer's instructions for employees to review prior to allowing any work to be performed.

Section 88.4 - Code of Iowa (2001) 88.4, Reference American National Standard ANSI MH16.1-1974, Section 1.4.2 : The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of not knowing the maximum weight limits the racks could support: (a)Warehouse - Permanent plaques, being not less than 50 square inches in area and showing the maximum permissible unit load in clear legible print, were not displayed on industrial steel storage racks. Among other methods, one feasible and acceptable abatement method to correct the hazard is to display approved plaques on all industrial steel storage racks.
