Section 88.4 - Code of Iowa (2001) 88.4: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by or caught-in hazards without the availability of an emergency stop: (a)Chain Conveyor, Paint Department - The start/stop buttons operating the chain conveyor are frequently by-passed electronically leaving employees without the availability of emergency stop devices as required. This violation was noted on or about 1/22/03. Among other methods, one feasible and acceptable means of abatement would be to revise conveyor controls such that they could not be by-passed in accordance with the applicable portions of ANSI B20.1, as outlined in Clause 5.11.2 of ANSI B20.1b - 1992.b
