Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
(a)  Plant Area;  Bobcat #4 worker operating a skid steer
loader without a functional parking brake and the seat belts
were excessively torn and would not extend or retract.  This
condition exposed worker to crushing injuries or death in
event of equipment rollover.
(b)  Plant Area;  Bobcat #3, Serial #503734082, worker
operating a skid steer loader without a functional parking
brake, seat belts that were excessively torn and would not
extend or retract.  This condition exposed worker to crushing
injuries or death in event of equipment rollover.
(c)  Plant Area; "Nissan" front end loader; Worker operating
loader without a functional parking brake and the seat belts
were not functional.  Additionally, the running lights were
inoperative and the equipment was being operated in a dimly lit
building.  This condition exposed worker to crushing injuries
or death from rollover, striking equipment or personnel.
(d)  Plant Area;  Worker operating a "Yale" lift truck in a
dimly lit area and the equipment did not have functional running
or brake lights.  This condition exposed workers to being struck
or operating striking equipment thereby exposing worker to
crushing injuries or death.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is verbally instruct and train employees through
demonstration as per the manufacturer's operations and maintenance
manual.
