Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to concussion and severe contusion injuries while depriming ammunition: a.The following conditions contributed to the hazard: (1) The ammunition depriming presses' collective systems were altered with out prior approval from engineering, safety and production departments, (2) The supervisor that altered the depriming machines chutes failed to review and assure that the alteration was as safe as the engineering replaced, (3) Operators failed to follow work instructions to check and clean depriming machines and their chutes, and to empty primer collection container, (4) The supervisor failed to verify that employees were following work instructions to check and clean depriming machines and chutes, along with seeing that the collective system was not overloaded with deprived primers. A feasible and useful means of abatement is for the employees and supervisors to follow established work practices and rules set forth; no employee whether they be management or labor shall change engineering of equipment manufactured without approval by engineering, safety and production departments.
