Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to:
On 11/11/98 and 12/8/98 there was a sudden and unexpected release
of flammable and toxic vapors and gasses from the rapid
decomposition of Cumene Hydroperoxide, a chemical used in the core
making process.  These incidents required an evacuation of the
plant.
Among other methods, one feasible and acceptable method to correct
this hazard is to follow manufacturer's recommendations  as listed
in Safe Handling of Organic Peroxides, ELF Autochem and information
listed in Factory Mutual Property Loss Prevention Data Sheet 7-80,
May 1998.  The specific  deficiencies noted on the site include but
are not limited to:
1. Excessive amounts of organic peroxide in the process area.  On
the day of the inspection the facility had a 55 gallon drum
connected to the feed system; only 1-5 gallons a week were used.
2. Storage of peroxide in the process area. On the day of the
inspection another unopened drum was stored in the process area.
All other organic peroxide must be stored in a properly constructed
and equipped storage room located in an isolated area.
3. No procedures to limit contamination of the peroxide were
established.n
