Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physcial harm to employees in that employees were exposed to: (a) being struck-by flying object(s) and/or contact with fire/explosion as the employee(s) were permitted to use non-relief fitting(s) such as a metal threaded water tank plug into the top of the string on the oil and gas well of the Oxford Oil Company, Lease # J-19-16, Permit No. 3199, Jackson TR 10, Noble Company. Among other methods, one feasible and acceptable abatement method to correct this hazard is providing instructions and utlizing proper fittings and/or devices on the top of the string that can safely relieve any pressure in the well.
