Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
Employees lifting steel coils using "C" hooks on which repair welds had
been made were
exposed to the hazard of being struck by loads falling from the hook, in
the event of hook
failure. The hooks were welded/repaired in house and tests were not made
following the repair
to ensure the safety of the hook.
Among other methods, one feasible abatement method is to ensure that the
provisions of
ANSI/ASME (American National Standards Institute/American Society of
Mechanical Engineers)
B30.20 safety standard for "Below the Hook Lifting Devices" or equivalent
are met prior to the
use of lifting devices which have been altered, modified, or repaired.
