Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to being struck or crushed by falling steel coils:
(a)  The "C" hook used in conjunction with the Schuette 25 ton crane had
severe wear at the
clevis where it attached to the crane hook and was being used to lift
steel coils weighing up to
or in excess of 46,000 pounds.  In addition to the existing damage, the
following requirements
were not met to ensure that employees were not exposed to failure or
overloading of special
lifting devices:
oThere was no determination made as to the rated load capacity of the "C
hook and the
capacity was not marked on the main structure so as to be visible to the
crane operator and the
unit was not proof tested.
oA nameplate or other permanent marking including at least (1) the
manufacturer's name
and address (or equivalent), (2) the serial number, (3) the weight of the
unit and (4) its rated
load was not displayed on the "C" hook.
oFrequent inspections were not conducted to detect deficiencies such as
structural
deformation, cracks or excessive wear on the "C" hook, so as to make a
determination as to
whether the conditions constituted a safety hazard.
oPeriodic inspections were not conducted for the "C" hook to detect
conditions such as
but not limited to excessive wear at the load supporting clevis.
Among other methods, one feasible abatement method is to ensure that the
provisions of
ANSI/ASME  (American National Standards Institute/American Society of
Mechanical
Engineers) B30.20 safety standard for "Below the Hook Lifting Devices" or
equivalent are met
prior to the use of any existing, new or repaired "C" hook or other
special load attachment
device.ent

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that:
(a)  Employees attaching or detaching the 24" filler saddles on the
recoiler for the Delta Slitter
were subject to being struck or crushed by shifting or falling saddle(s)
weighing approximately
200 pounds each due to the fact that the threads inside some of the bolt
holes were stripped,
resulting in fewer bolts holding the saddle(s) in place than is required
by the manufacturer.
Among other methods, one feasible and acceptable means for abating this
condition include
repairing and retapping the bolt holes to bring them into compliance with
manufacturer's
specifications; implementing a preventative maintenance program which
complies
with the
manufacturer's specifications; and implementing a training program for
operators and helpers
which enables them to (1) recognize potential hazards associated with
attaching and when to
remove saddles from service and report problems, (2) ensure that bolts are
not over-torqued, and
(3) ensure that bolts are properly and completely removed before the crane
is used to pull the
saddle.
