Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
a.On and about March 1, 2000, in the foundry area, employees were exposed
to hazards
of falling or being crushed while boarding, attempting to board or waiting
to board
cranes in that, after boarding light systems had been installed and made
operational on
cranes, such as, but not limited to, the ladle crane, the employer did not
establish,
communicate to effected employees, and enforce procedures for the boarding
of such
cranes while in operation.  Said procedures would cover the safe boarding
of cranes by
employees such as, but not limited to, crane operators who relieve other
crane operators
for shift changes and breaks, and maintenance personnel, yard men and
other
employees
who board the cranes for adjustments, repairs and/or inspections.
Among other methods, a feasible and acceptable method to correct this
hazard is for the
company to put into writing, conduct training on and enforce a program for
affected
employees which includes, at a minimum, the following procedures:
-  A method of notifying the crane operator which includes, but is not
necessarily
limited to, the installation of a boarding light and/or buzzer that is
turned on from
the catwalk.
-  A procedure to stop and red light the crane, and  to establish a signal
that an
employee wishes to board the crane.
-  The boarding of cranes only at boarding stations and a procedure to
ensure the
boarding employee that it is safe to do so.
-  A procedure, such as signals, that indicates that the employee is
safely off the
crane before the operation of the crane can proceed.
-  A procedure to board cranes safely in the event that the crane is
disabled and can
not be moved to a boarding station, such as a harness system for fall
protection
and a lockout procedure for electrical and mechanical hazards.
