Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to becoming entangled in a rotating rewind shaft:
(a)  Location:  Rewinding Station - Building # 13
On 02/21/95, an employee received serious injuries while applying adhesive
tape to the
core of the front rewind rotating shaft by engaging the clutch. His loose
jacket got caught
on the tape, pulling him into the machine and wrapping him around the
spindle.
Among other methods, acceptable abatement would be to develop and
implement proper
procedures and to train the operator to use glue or adhesive tape only
after disengaging
the front roller from the flywheel clutch. A policy should be implemented
on the type
of clothing allowed to be worn in areas where there are rotating parts.ng
