employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a)  Location:  Production Area;
Employees were exposed to a back-flash fire hazard.  On or about 6/23/95,
employees
were unprotected whenever back-flash fires shot out from the area of the
damper actuator
located under the Burns, Model 24-30 Fry-O-Matic machine that was not
maintained
according to the manufacturer's instruction.
Among other methods, one feasible and acceptable abatement method is to
make sure that
all maintenance workers read and understand the operating and maintenance
instructions
for the Fry-O-Matic machine and follow the procedures to include using
original
equipment parts or the equivalent.
