On or about January 22, 1996, an employee was working from a snorklelift,
changing light bulbs, when his snorklelift was struck by a moving bridge
crane,
causing it to tip over and kill the employee.
Employees were told to take the radio operating controls with them, but
sometimes they did not.
The employer shall insure that radio controls are carried with the
employee on
the snorklelift working in the vicinity of the overhead bridge cranes.
