29 CFR 1910.132(a):  Personal protective equipment, including personal
protective equipment
for the eyes, face, head, and extremities, protective clothing,
respiratory devices, and protective
shields and barriers, were not provided when necessary by reason of
hazards of processes or
environment, chemical hazards, radiological hazards, or mechanical
irritants encountered in a
manner capable of causing injury or impairment in the function of any part
of the body through
absorption, inhalation or physical contact:
(a)An employee was observed welding on the top of a carbody and was
exposed to
falls of 10 to 22 feet onto the concrete below and was not protected by a
personal
fall arrest system in that:
1)while operating a Ransome positional straight line welder, the employee
did not always wear the manufacturers specified personnel restraint system
while seated in the unit;
2)the employee was using a body belt while exposed to fall
equipment
for the eyes, face, head, and extremities, protective clothing,
respiratory devices, and protective
shields and barriers, were not provided when necessary by reason of
hazards of processes or
environment, chemical hazards, radiological hazards, or mechanical
irritants encountered in a
manner capable of causing injury or impairment in the function of any part
of the body through
absorption, inhalation or physical contact:
(a)An employee was observed welding on the top of a carbody and was
exposed to
falls of 10 to 22 feet onto the concrete below and was not protected by a
personal
fall arrest system in that:
1)while operating a Ransome positional straight line welder, the employee
did not always wear the manufacturers specified personnel restraint system
while seated in the unit;
2)the employee was using a body belt while exposed to fall
arrest conditions;
3)the portable anchor point system in use was a magnetic
device normally used for lifting loads of steel;
4)the magnetic device was not designed as an anchor point
system and could be adversely impacted by use near
welding operations;
5)employees were exposed to falls during the movement and
use of the magnetic device.
Certification, with date and method of abatement required.
Certification of abatement is required by 29 CFR Part 1903 and is to be
submitted within ten (10)
calendar days after the abatement date.ment
