Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that:
Employees are exposed to crushing hazards when conducting lifts of the
blast hole drill
platform as a result of the following:
(a)A four legged chain sling, utilizing eye bolts was overloaded when
lifting
a platform at a 60 degree, from vertical, lift.
(b)A four legged chain sling, utilizing shoulder bolts was overloaded when
lifting a platform at a 60 degree, from vertical, lift.
Among other methods, feasible and acceptable means to correct these
hazards are to use
a safer alternate method such as:
(a)Following the manufacturer's instructions and limitations.
(b)Ensure that the eye/shoulder bolts are rated for the weight being
lifted in
accordance with ANSI/ASME B18.15.
(c)Provide employees with swivel hoist rings rated for the loads being
lifted.
Certification, and documentation/evidence of abatement is required.
Certification of abatement is required by 29 CFR Part 1903 and is to be
submitted within ten (10)
calendar days after the abatement date.ment
