General-duty citation text

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to hazard of being struck by falling load in the event of
lifting device failure:
a)  Paper Slitting Area - Employees used two gates hi-power
II v-belts as lifting devices to attach paper rolls up to
28" wide to the hook of an overhead hoist.  No rolls were
then hoisted onto the paper slitter using the v-belts as
slings.  Employees were thus exposed to injury in the event
of v-belt failure resulting from v-belts being used in a
manner for which they were not designed, condition last
existed about January 1993.
Among others, one feasible and acceptable abatement method
to abate the hazard is to discontinue use of v-belts in this manner
and to supply proper slings to perform the job.
