Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazard from a falling load: a) Number Four (4) Skimmer House - A continuous splice sling was made from a 1/2 inch wire rope. The splice was with two (2) wire rope clips and the wire rope was run through two (2) eye bolts on top of the spinner motor, on or about April 6, 1993. Among other methods, one feasible and acceptable abatement method to correct this hazard is to only use manufactured slings when hoisting equipment and not pull on the sides of eye bolts.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazard from a falling hopper because of rigging failure: a) Silo, Line 11 Derrick Screen - The large steel hopper was suspended by four (4) 1/2 inch wire ropes. There field fabricated slings only had two (2) wire rope clips per eye and some of them were on backwards. These slings were connected to shoulderless eye bolts and the slings were pulling on the side of the eye bolt, on or about April 14, 1993. Among other methods, one feasible and acceptable abatement method to correct this hazard is to use the proper amount of wire rope clips (3 each for 1/2 inch wire rope) on each eye and place the saddle on the line end of the rope. The eye bolts used should be the shoulder type.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to multiple serious upper body injuries from a falling reactor flusher: a) Building One, Reactor Floor - The reactor flushing unit was hoisted by placing the hoist hook through a eye that was formed by bending a piece of "all thread". This connection was backed up by a piece of 1/4 inch steel chain that was held together by a bolt, on or about April 5, 1993. Among other methods, one feasible and acceptable abatement method to correct this hazard is to only use drop forged eyes when hoisting by the ey, never use a bolt as a fastener for a chain sling and only use alloy steel chain.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a potential 45 foot fall hazard if the main boom pin shears: a) Occidental Chemical - The grease fitting on the right side of the main boom pivot pin was broken and the bushing on that side of the pin was smashed out of round. Lack of lubrication will cause the main boom pivot pin to shear. Exposing employees to a maximum 45 foot fall hazard, on or about March 30, 1993. Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the manufacturer recommendation of lubricating the two (2) lubrication points on the main boom pivot pin every 10 hours of use.

The platform was lowered into a 15 foot deep clarifier tank to cut blades off the rotating shaft, on or about March 30, 1993. Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the crane and derrick suspended personnel platform standard contained in 29 CFR 1926.550(g)(1)-(8).

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a twelve (12) foot, eight (8) inch fall hazard. a) The Hi-Jacker telescoping work platform, number 836-5, was 12 feet, 8 inches high and the chain mid rail was missing on 836.7. There was no mid rail on the 12 foot, eight inch high platform, the wire rope used to hoist the platform was very rusty, smashed from not properly wrapping it on the drum and the wire rope end that holds the rope on the drum had slipped very close to the end of the rope and a clip was not used on the drum to secure the hoist rope, on or about April 6, 1993. Among other methods, one feasible and acceptable abatement method to correct this hazard is to install proper railing, replace the hoist rope and properly fasten the hoist rope to the drum.
