29 CFR 1910.137(c)(2)(vi): Insulating equipment was not stored in such a location or in such a manner as to protect it from light, temperature extremes, excessive humidity, ozone, and other damaging substances and conditions. Employees engaged in, but not limited to, the cell renewal process in the H-3 Cell room were exposed to electrical shock hazards in that the rubber electrical insulating gloves were not stored and maintained to prevent damage from environmental factors.

29 CFR 1910.137(c)(2)(vii): Protective gloves were not worn over insulating gloves. Employees engaged in, but not limited to, the cell renewal process in the H-3 cell room were exposed to electrical shock hazards in that dielectric insulated rubber gloves were utilized without any over protection of the glove.

29 CFR 1910.137(c)(2)(vii)(D): Insulating gloves that have been used without protector gloves were reused without testing under the provisions of paragraphs (c)(2)(viii) and (c)(2)(ix) of this section. Employees engaged in, but not limited to, the cell renewal process in the H-3 cell room were exposed to electrical shock in that electrically insulated rubber gloves were allowed to be used without over protectors and re-used without proper testing.
