Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to falling load due to failure of wire rope and
the lack of markings
on the pendant controls:
a) Furnace Area - Employees operated a Electro Lift Shaw box 1 ton hoist
with two
parts of line on an underhung crane .  The wire rope on the hoist was
severely
damaged.  The rope showed numerous broken wires on the rope from the
dead-end
connection to the load block when set on the floor.  Numerous broken
wires were
noted in one lay of rope with 6 to 8 breaks in one strand in that lay.
b) Degreaser area - Pendant controls were not marked on the Cleveland
Tramrail
overhead underhung crane.
c) 6200 Department - Pendant controls were not marked on the on the
electro lift one
ton overhead underhung crane.
ABATEMENT NOTE: Among others one feasible and acceptable abatement method
is to
replace the wire rope on the crane and to establish an inspection program
by a competent
person for all underhung cranes based on the criteria set forth in
American National
Standards Institute (ANSI) B30.11 Monorail Systems and Underhung Cranes,
latest edition.
