29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:  (a)  Production - On or about March 3, 2016, employees engaged in servicing and maintenance activities, such as replacing the pads on the jaws of the trolley were not trained in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  (b) Production - On or about March 3, 2016, employees whose job requires them to operate or use a machine or equipment on which servicing or maintenance is being performed under lockout or tagout, or whose job requires him/her to work in an area in which such servicing or maintenance is being performed were not instructed in the purpose and use of the energy control procedure.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a)  Production - On or about March 3, 2016, specific procedures were not documented for or utilized to control the hazardous energy sources on the Nitinol Draw Bench, including but not limited to hydraulic and pneumatic, during servicing and maintenance activities, such as replacing the pads on the jaws of the trolley.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) Plant 1 - Draw Bench WC#1001 - On or about March 3, 2016, employees were exposed to hazards created by nip points on the drawbench where trolley wheels meet the rail on which it traverses horizontally.    (b) Plant 1 - Draw Bench WC#1013 - On or about March 3, 2016, employees were exposed to hazards created by nip points on the drawbench where trolley wheels meet the rail on which it traverses horizontally.    (c) Plant 1 - Draw Bench WC#1006 - On or about March 3, 2016, employees were exposed to hazards created by nip points on the drawbench where trolley wheels meet the rail on which it traverses horizontally.    (d) Plant 1 - Draw Bench WC#1108 - On or about March 3, 2016, employees were exposed to hazards created by nip points on the drawbench where trolley wheels meet the rail on which it traverses horizontally.    (e) Plant 1 - Draw Bench WC#1109 - On or about March 3, 2016, employees were exposed to hazards created by nip points on the drawbench where trolley wheels meet the rail on which it traverses horizontally.    (f) Plant 1 - Draw Bench WC#1112 - On or about March 3, 2016, employees were exposed to hazards created by nip points on the vertical drawbench where trolley wheels meet the rail on which it traverses horizontally.
