Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish
employmnent and a place of employment which were free from recognized
hazards that were causing or likely
to cause death or serious physical harm to employees in that employees
were exposed to the hazard of concrete
products such as but not limited to Connecticut State Highway Department
Standard Double Type 2 Curb Top
from unexpectedly falling from the metal forms while being rotated 180
degrees:
On or about August 15, 2002, a Connecticut State Highway Department
Standard Double Type 2 Curb Top
fell from it's metal form while being elevated four to four and a half
feet above the ground in the process of
being rotated 180 degrees with the assistance of an employee.
Among other methods, feasible and accept able abatement methods to correct
this hazard would be to install
engineered metal bars along the length of the metal form on the open face
top with positive locking
mechanisms to prevent the concrete products from falling out of the form.
